For all other concerns with insurance please call Employee Plans customer service at 625-7500.

For Cobra, payment booklets for retirees, or certificate of coverages forms please call Jill Rosenogle at 625-7253.

EACS does not offer open enrollment. 

To obtain insurance coverage you must have a qualifying event such as:  death, divorce, loss of other coverage, birth/adoption of child, or marriage.  You will have 30 days from the event date to make changes to your plan.  Please contact Leslie Reisgies at 260-446-0100 ext. 1051.

New employees hired after August 1, 2010, EACS follows the spousal carveout provision.   This means that if you spouse is eligible for insurance coverage through his/her employer they would not be allowed to be covered on our plan as a primary.
