EACS Voluntary Life Insurance

East Allen County Schools has an optional employee-paid, voluntary term life coverage available at group premium rates.  The Voluntary Life Insurance coverage is offered by American United Life Insurance Company® (AUL), an Indiana based insurance company located in downtown Indianapolis.   To view the Voluntary Term Life Insurance Policy click here and to view the Voluntary Term Life Coverage for EACS Employees click here.

Group Premium Rates - The premium is based on low group premium rates. Life insurance coverage is also available for eligible dependents based on the amount of coverage you approved for yourself.
Flexibility - Select the amount of coverage you need, between $10,000 and $100,000, or up to 5 times annual salary.  By covering yourself as an employee, you'll have the opportunity to cover your eligible dependents as well.
Guaranteed Issue Amounts - As an employee, you will be eligible to apply for up to $ 100,000 of voluntary term life insurance coverage regardless of your current health status. (This amount of coverage is only offered if you apply during the initial enrollment period. You will not be able to wait a few months and then apply without medical underwriting.)
Payroll Deduction - Premiums are paid using the convenience of payroll deduction.

Initial Enrollment:  New employees are allowed to participate in the Voluntary Life Insurance Program if they are regularly scheduled to work at least 19 hours per week during the school year.  The initial enrollment must occur within 30 days of their intial hire date.  The enrollment form is available through the link below.

Late Enrollment:  On an annual basis during the month of December, employees who had previously declined, have the opportunity to request the Voluntary Term Life Insurance. Because this is considered to be an annual late enrollment, there is no guarantee of insurance and medical underwriting insurability is required.  The enrollment form is available through the link below.

Annual Opportunity to Increase Life Value:  An opportunity to increase your current voluntary life insurance coverage without Evidence of Insurability.  This is offered to employees who are currently participating in the East Allen County Schools Voluntary Term Life Insurance program.  Participants are eligible to increase their coverage by the greater of 10% of their current amount or $10,000 to the contract maximum without answering any medical questions.  If an employee is interested in increasing his/her life amount then the "Guaranteed Increase Benefit Offer" form (sections: Employee; Date of Birth; Current Life Amount; New Life Amount; Date and Sign the Document must be completed.  Completed forms must be submitted to Susan Bandt by December 17, 2012.
